Report: Redundancies, safeguards failed on Deepwater Horizon

(NEW YORK) — A series of redundancies and safeguards aboard the Deepwater Horizon failed on the day it exploded, the New York Times reported. The crew reported that there was a lack of communications and a virtual paralysis during the nine minutes of crisis. A failure to train for the worst, plus the complexity of the rig, contributed to the general breakdown of all safety backups, which contributed to the loss of life.
